This One Mistake Puts GLP-1 Users at Risk of Fatigue, Weakness, and Weight Regain

If you’re using a GLP-1 medication like Wegovy, Ozempic, or Mounjaro, or considering using them, you’re not alone. One out of every three people who come to me for nutrition support is either currently taking these medications or is curious if they are for them. These medications were originally developed to manage type 2 diabetes; however, GLP-1s are now widely used to support weight loss and regulate appetite. Many of my clients—especially those in midlife—are seeing significant improvements in their weight and health markers with the help of these medications. It’s also important to note that many people can still achieve weight loss with lifestyle interventions alone. Not everyone needs a medication.

Those who are on medication make a common mistake I see often that puts all of their progress at risk: relying solely on the medication without a change to their lifestyle habits. The immediate signs include fatigue and tone and muscle loss, which will ultimately lead to weight regain. Put it this way, if you aren’t fueli8ng your body, you will be too tired to move, prepare meals, and exercise, and your results will not last.

What Are GLPs?

From the most basic sense, GLP-1s (glucagon-like peptides) are a key hormone in your body that help regulate blood sugar, insulin, and appetite. Our body naturally produces glucagon-like peptide in the intestines in response to eating, allowing it to regulate blood sugar, appetite, and digestion by signaling fullness, increasing insulin, and decreasing glucagon. Can you can support your body’s natural GLP-1 production through dietary choices? Yes, you can. And in healthy people where blood sugar and insulin regulation is efficient, medication isn’t needed. Consuming foods such as soluble fiber, protein, and healthy fats are beneficial to this process and so is consistent exercise.

Medications Offer Therapeutic Effects for the Treatment of Disease

Most people associate GLP-1 medications with weight loss; however, these medications are the most powerful tools for treating diseases including diabetes, cardiovascular disorders, and neurodegenerative diseases, by controlling blood glucose and improving metabolic syndrome. For someone already experiencing these conditions, a medication might be warranted. Let’s explore what the most popular ones are:

  • Semaglutide (Ozempic, Wegovy)
  • Tirzepatide (Mounjaro, Zepbound – this one also targets GIP, another hormone)
  • Liraglutide (Saxenda, Victoza)

These medications work by slowing the rate at which your stomach empties, increasing insulin sensitivity, and reducing hunger. Many people find they simply aren’t interested in eating, or they feel nausea that makes then makes eating completely unappealing. However, the body still needs the nutrients in foods that are essential to life. One patient described the effects as,

“Nothing sounds appetizing to me. Sometimes some junk food does, but it’s hard for me to decide on eating a food when I don’t have an appetite.”

The Side Effects I See Most Often

As a dietitian working closely with people on these medications, I can tell you: appetite suppression and nausea are real—and they’re often misunderstood. While they may help reduce calorie intake, they can also make it harder to nourish your body properly.

Common experiences I see include:

  • Eating too little to meet basic nutrient needs
  • Inadequate protein intake – or too much protein and not enough diversity in the diet
  • Low energy and fatigue
  • Digestive issues (constipation, reflux, bloating)
  • Fluid losses, risk of dehydration
  • Muscle loss due to insufficient calories and strength training

And for some, GLP-1s may become less effective over time, which makes it even more important to build habits that don’t depend on medication alone.

When GLP-1s Cause Discomfort or…Just Don’t Work

While many people respond well to these medications, they may not be the right fit for everyone. Some people have to stop using the medication because they experience significant gastrointestinal distress, which may include:

  • Chronic constipation
  • Persistent bloating or early fullness
  • Nausea and persistent acid reflux

This is where personalized support matters. I help clients reduce these symptoms by adjusting their food texture, fiber intake, hydration strategies, and meal timing—ensuring they still get what they need, even when their intake is limited.

Why Nutrition and Muscle Matter—A Lot

GLP-1 medications can support meaningful weight loss—but without a clear plan, it’s not always fat that’s being lost. You could be losing muscle, especially if you’re not actively protecting it through diet and strength training. This is especially concerning in our 40s, 50s, and beyond, when muscle becomes harder to rebuild.

Loss of muscle mass is tied to:

  • Slower metabolism
  • Higher risk of weight regain
  • Fatigue and weakness
  • Poorer glucose control
  • Greater risk of injury or physical decline

That’s why the most important thing you can do alongside your GLP-1 journey is build the right foundation of nutrition and movement.

Prioritize Protein and Nutrient Density

No matter if you are taking a medication or looking to lose weight, every bite needs to count. When we prioritize protein, I don’t mean overload your diet with it. Too often I see a protein-only diet with very little fiber and healthy fats.
